Choosing between Route 1 and Route 2

  1. If you have access to ID such as a birth certificate, passport, photo driving licence, NHS medical card, PASS card or Home Office Card apply using Route 1
  2. If you don’t have ready access to any of the ID listed above, apply using Route 2

ROUTE 1

The easiest way to get a CitizenCard is to show ID to a verifier.

1. If the verifier knows you personally the ID can be either a birth certificate, an official copy of a birth certificate, Home Office Card or an NHS medical card.

2. If the verifier does NOT know you personally the ID must be either a photo driving licence, passport, national ID card or another PASS-hologram card eg. Connexions or Prove It!.

The verifier must see the original ID and must then countersign a photocopy of that ID which you then post to us with your completed application.

*If your name has changed evidence of this must also be provided eg. marriage certificate, change of name deed.  The verifier must see the original document and must countersign a photocopy of it, which should be posted with your application and countersigned ID.

Any person in our permitted list of verifiers over 25 and in full time employment can act as a Route 1 verifier provided we can contact them on a non-mobile phone at their place of work during office hours at a registered business address.  Verifiers must not be related to the applicant by birth or marriage neither should they be in a personal relationship together nor live at the same address.  They must also not be self employed or work from home.

ROUTE 2

Your CitizenCard application can be verified by one of the following persons, provided that they know you and can confirm your details against their official records held at their place of work:

Your doctor/dentist/nurse/administrator – checking NHS records

• A teacher/tutor/administrator at Your school/college/university

Your social worker/ Your certified solicitor

• A bank / building society staff member at Your branch

Your Connexions adviser / Your prison or probation officer
